    Background:

    Purpose of the Study:

    • To investigate cell-mediated immunity in elderly patients with negative tuberculin skin tests.
    • To assess T cell populations and function in this specific geriatric cohort.

    Main Methods:

    • Routine tuberculin skin testing on 880 hospitalized geriatric patients.
    • Selection of 54 patients with negative skin tests and no interfering diseases.
    • Application of a battery of tests for cell-mediated immunity, including DNCB sensitization, T cell percentage, in vitro mitogen response (PHA), and leukocyte migration inhibition factor (LIF) production.

    Main Results:

    • Elderly patients negative to 100 i.u. tuberculin rarely sensitized to other thymus-dependent antigens.
    • Low sensitization rate to DNCB (20%) observed in the selected group.
    • Reduced peripheral blood T cell percentage (36%) and poor in vitro response to PHA.
    • 25% of participants showed impaired LIF production in vitro.
    • In vitro antigen response correlated well with skin test results.

    Conclusions:

    • Suggests an absolute decrease in circulating T lymphocytes in some elderly individuals.
    • Indicates a likely functional impairment of T cells in a subset of this population.
    • Highlights the importance of comprehensive immune assessment in the elderly.
