Background:

Renal leiomyomas are rare benign mesenchymal tumors originating from kidney smooth muscle cells.
They are typically located in the renal capsule and can be asymptomatic when small.

Observation:

A case of a giant renal leiomyoma (17x15x13 cm) in a 20-year-old female presenting with abdominal discomfort and a palpable mass.
Imaging revealed a large, exophytic, solid-cystic mass arising from the upper pole of the left kidney.

Findings:

Histologically, renal leiomyomas show intersecting fascicles of spindle cells with actin and desmin immunoreactivity.
The large size and complex nature of this tumor were confirmed via CT scan.

Implications:

Giant renal leiomyomas, though rare, require surgical intervention due to potential symptoms and diagnostic limitations of biopsy.
This case highlights the importance of considering leiomyoma in the differential diagnosis of large renal masses.
