Summary

Lateral retroperitoneoscopic adrenalectomy (LRA) is a safe and effective procedure for adrenal tumors. While successful for most cases, large tumors and complex conditions may require open surgery.

Background:

  • Lateral retroperitoneoscopic adrenalectomy (LRA) is increasingly adopted by general surgeons due to direct adrenal gland access.
  • Management of large adrenal tumors via LRA remains a subject of debate.

Purpose of the Study:

  • To evaluate the feasibility, safety, and outcomes of LRA for adrenal tumors.
  • To compare outcomes between patients with large (>5 cm) and small (≤5 cm) adrenal tumors.

Main Methods:

  • Retrospective analysis of 89 consecutive LRA procedures performed between December 2011 and April 2015.
  • Data collected included conversion rates, operative time, blood loss, hospital stay, and complication rates.
  • Patients were stratified into groups based on tumor size (≤5 cm vs. >5 cm) for comparative analysis.

Main Results:

  • Low conversion rate (1.1%) and zero mortality were observed.
  • Mean operative time was 107.4 min, mean blood loss 33.15 ml, and mean hospital stay 4.7 days.
  • Significant differences were found in operative time, hospital stay, early postoperative complications, and conversion rates between large and small tumor groups.

Conclusions:

  • LRA is a feasible, effective, and safe approach for adrenalectomy, offering direct access advantages.
  • Malignancy, large tumor size, bilateral disease, or concurrent intra-abdominal pathology may necessitate alternative surgical approaches.
