Periostitis ossificans (PO) in children can be treated with endodontic intervention, avoiding tooth loss. This less invasive approach offers a successful alternative for managing this odontogenic infection.

- Radiographic and CT imaging revealed characteristic onion-skin periosteal bone proliferation in the mandible.

- Endodontic treatment of tooth 36, including root canal preparation and calcium hydroxide medication, was performed.
- Complete remission of periostitis ossificans was achieved within 14 months post-treatment.
Implications:
- Endodontic intervention presents a less invasive treatment option for pediatric periostitis ossificans.
- This approach can effectively manage the condition and minimize the risk of tooth loss in pediatric patients.
