Permanent pacemaker implantation after cardiac surgery: Optimization of the decision making process

Early permanent pacemaker (PPM) implantation after cardiac surgery is linked to higher late PPM dependency. Delaying PPM implantation until after postoperative day 7 may improve outcomes and prevent unnecessary procedures.

Background:

Purpose of the Study:
- To investigate whether PPM implantation timing and specific conduction disturbances predict late pacemaker dependency and AV conduction recovery after cardiac surgery.

- A cohort of 177 patients who received PPM implantation after cardiac surgery were followed in an outpatient clinic.
- Outcomes assessed included AV conduction recovery and PPM dependency (defined by absence of intrinsic rhythm in VVI mode at 40 bpm).

- Of 145 patients with follow-up data, 81 (56%) were not PPM dependent.
- PPM implantation within 6 days post-surgery predicted both not being PPM dependent (OR, 5.40) and AV conduction recovery (OR, 4.96).
- Sinus node dysfunction as an indication for PPM predicted not being PPM dependent (OR, 6.59).
Conclusions:
- Implanting a permanent pacemaker (PPM) on postoperative day 7 is recommended.
- This timing may prevent unnecessary PPM implantations and reduce prolonged hospital stays.
- Early PPM implantation (≤6 days) is associated with higher rates of late pacemaker dependency and lower rates of AV conduction recovery.
