Background:

  • Drug-induced hemolytic anemia is a rare but serious adverse reaction.
  • Pediazole, a combination of erythromycin ethylsuccinate and sulfisoxazole, is used to treat bacterial infections like tonsillitis and otitis media.

Observation:

  • A 3-year-old female presented with severe hemolytic anemia while on Pediazole therapy.
  • Standard in vitro tests for drug-dependent antibodies were negative.
  • A specialized in vitro assay using erythromycin-coated red blood cells demonstrated drug-dependent hemolysis.

Findings:

  • This case represents the second documented instance of immune hemolytic anemia linked to erythromycin.
  • It is the first reported case where in vitro drug-dependent hemolysis was successfully demonstrated for erythromycin.

Implications:

  • This finding highlights the potential for erythromycin to cause immune hemolytic anemia through a drug-dependent mechanism.
