Summary

Deep brain stimulation (DBS) offers long-term benefits for severe obsessive-compulsive disorder (OCD). Rechargeable devices are well-tolerated and cost-effective, supporting DBS as a viable treatment option.

  • Interest is growing in DBS for neuropsychiatric conditions like obsessive-compulsive disorder (OCD).
  • Clinical data on DBS for OCD remains limited.

Purpose of the Study:

  • To evaluate the long-term efficacy and tolerability of DBS for severe OCD.
  • To assess the impact of rechargeable impulse generators (IPGs) in OCD patients.

Main Methods:

  • Retrospective study of nine severely affected OCD patients.
  • Ventral capsule/ventral striatum DBS implantation.
  • Average follow-up of 54.8 months.

Main Results:

  • Majority of patients showed significant improvement in OCD and depressive symptoms.
  • Six patients achieved periods of OCD remission, with four responders needing over 12 months.
  • Higher stimulation settings were required, increasing IPG battery demand.
  • Rechargeable IPGs were well-tolerated and did not worsen OCD or anxiety symptoms.

Conclusions:

  • DBS can be successfully implemented for long-term benefit in severely affected OCD patients in real-world settings.
  • Optimal patient selection and programming are crucial for DBS in OCD.
  • Rechargeable IPGs are a cost-effective and well-tolerated option for this patient population.
