    Area of Science:

    • Microbiology
    • Clinical Diagnostics
    • Biochemistry

    Background:

    • Carbapenemase-producing Enterobacteriaceae (CPE) are a significant global health threat.
    • Rapid and accurate detection of carbapenemase activity is crucial for effective patient management and infection control.
    • Existing diagnostic methods can be slow, complex, or lack sensitivity.

    Purpose of the Study:

    • To develop and validate a rapid, sensitive, and specific carbapenemase detection assay.
    • To provide an objective and cost-effective diagnostic tool for clinical laboratories.

    Main Methods:

    • Development of a carbapenemase test utilizing imipenem's inhibitory effect on non-carbapenemase beta-lactamases.
    • Incorporation of a fluorescent beta-lactamase substrate for bacterial isolates.
    • Assay validation for sensitivity, specificity, and turnaround time.

    Main Results:

    • The developed carbapenemase test demonstrated 100% sensitivity and 100% specificity.
    • Objective results were obtained within 10 minutes of sample processing.
    • The assay requires only a single mixing step and is inexpensive to perform.

    Conclusions:

    • This novel carbapenemase test offers a rapid, accurate, and cost-effective solution for identifying carbapenemase-producing bacteria.
    • The assay's simplicity and speed facilitate timely clinical decision-making and infection control strategies.
    • This diagnostic tool has the potential to significantly improve the management of infections caused by carbapenem-resistant organisms.
