SBAR I: Understanding the Concept

Effective communication among healthcare professionals during hand-off reporting is essential to delivering safe and continuous patient care. Common professional interactions include reports to healthcare team members, hand-off, and transfer reports. Nurses routinely report information to other healthcare team members and also urgently contact healthcare providers to report changes in patient status.
Standardized methods of communication have been developed to ensure that information is...

Types of Reports I: Hand-off Report

A hand-off report, also known as a change-of-shift report, is a crucial nursing process that ensures the smooth transition of patient care responsibilities between nursing staff.
Following are the key components and categories of hand-off reports:

Area of Science:

  • Emergency medicine
  • Healthcare quality and safety
  • Health services research

Background:

  • Emergency medical dispatch (EMD) plays a critical role in patient triage.
  • Existing incident reporting systems may not capture all patient harms.
  • Trigger tools offer a potential method for identifying missed errors.

Purpose of the Study:

  • To evaluate the utility of trigger tools in identifying triage errors made by EMD nurses.
  • To characterize patients referred to non-emergency care by EMD.

Main Methods:

  • Observational study conducted in two Swedish regions.
  • 1089 adult patients referred to non-emergency care by EMD nurses were included.
  • Primary outcome: emergency department visit within 7 days; secondary outcomes included related visits, higher-level care provision, and hospital admission.

Main Results:

  • 24% of patients visited an emergency department within 7 days; 80% of these visits were linked to the EMD contact.
  • Elderly patients and those referred to other providers had a higher likelihood of ED visits.
  • Six avoidable patient harms were identified, missed by existing systems, highlighting potential for improved compliance with decision support.

Conclusions:

  • Global Trigger Tool-based reviews can uncover patient harms missed by incident reporting in EMD.
  • Increased adherence to EMD decision support systems has the potential to enhance patient safety.
