Summary

Small dense LDL cholesterol (sdLDL-C) determination is useful for cardiovascular risk stratification. This easy method correlates with LDL particle size and concentration, aiding in uncovering residual cardiovascular risk.

Background:

  • Cardiovascular disease prevention primarily relies on lowering LDL-C, yet residual ischemic event risk persists.
  • Increased small dense LDL (sdLDL) particles are linked to this residual risk.

Purpose of the Study:

  • To evaluate the utility of sdLDL cholesterol concentration (sdLDL-C) measured by a simple precipitation method.
  • To assess how clinical and biochemical factors, analyzed by NMR, influence sdLDL concentration.

Main Methods:

  • sdLDL-C and NMR lipid profiles were analyzed in 85 male samples.
  • Pearson coefficients assessed associations, and multivariate regression identified NMR variables influencing sdLDL-C.

Main Results:

  • Strong correlations were found between sdLDL-C and LDL particle number (LDLLDL-P) (r=0.687) and LDL size (LDL-Z) (r=-0.603).
  • Multivariate regression explained 56.8% of sdLDL-C variation, with BMI, ApoB, triglycerides, FFA, and LDL-Z showing significant contributions.
  • ApoB and LDL-Z were most influential; a 1nm increase in LDL-Z decreased sdLDL-C by 126 nmol/L.

Conclusions:

  • A clear association exists between sdLDL-C, LDL-Z, and LDL-P.
  • LDL-Z and ApoB significantly influence sdLDL-C.
  • The easy sdLDL-C determination method can aid in cardiovascular risk stratification and identifying residual risk.
