    Summary

    Serial neurologic exams in children treated for hyperactivity showed high variability in soft neurologic signs. This variability makes these signs unreliable for assessing drug effects or treatment improvement.

    Background:

    • Hyperactivity is a common concern in children.
    • Pharmacologic treatments are frequently used to manage hyperactivity.
    • Soft neurologic signs are sometimes used in clinical assessments.

    Purpose of the Study:

    • To evaluate the reliability of specific soft neurologic signs in children undergoing pharmacologic treatment for hyperactivity.
    • To determine if these signs can accurately reflect treatment effects.

    Main Methods:

    • Serial neurologic examinations were conducted on 44 children with hyperactivity over five occasions.
    • Three specific "soft" neurologic signs were analyzed: heel gait, toe gait, and diadochokinesis.
    • Data were analyzed to assess individual response variability and potential treatment interactions.

    Main Results:

    • A significant degree of variability in soft neurologic sign responses was observed within individual children.
    • No evidence of interaction was found between the pharmacologic treatment and the observed neurologic sign responses.
    • The high variability indicated that these signs were not sensitive indicators of drug effect.

    Conclusions:

    • The analyzed soft neurologic signs (heel gait, toe gait, diadochokinesis) demonstrate considerable intra-individual variability.
    • These signs are unlikely to be reliable measures for assessing the efficacy of pharmacologic treatments for hyperactivity.
    • The diagnostic or therapeutic assessment utility of these specific soft neurologic signs in this context is questionable.
