18F-PSMA 1007 in Suspected Renal Cell Carcinoma

F-PSMA 1007 PET/CT shows high tracer uptake in renal cell carcinoma, unlike prior studies. This imaging may aid in preoperative diagnosis, staging, and theranostic evaluation of kidney cancer.

Background:
- Renal cell carcinoma (RCC) diagnosis and staging are critical for treatment planning.
- Fluorodeoxyglucose (F-FDG) Positron Emission Tomography/Computed Tomography (PET/CT) is used in oncology but has limitations for certain tumor types.
- Prostate-specific membrane antigen (PSMA) PET/CT is emerging as a valuable tool in cancer imaging.
Observation:
- A 60-year-old male with suspected right RCC underwent both F-FDG PET/CT and Fluorine-18 (F)-PSMA 1007 PET/CT.
- F-PSMA 1007 PET/CT demonstrated intense tracer uptake in the primary right kidney tumor and all lung nodules.
- This intense uptake in the primary renal tumor with F-PSMA 1007 contrasts with findings from some previous pilot studies using Gallium-68 (Ga)-PSMA.
Findings:
- F-PSMA 1007 PET/CT revealed significant tracer avidity in the primary RCC.
- Consistent and intense tracer accumulation was observed in metastatic lung nodules.
- The primary renal tumor itself showed notable tracer concentration with F-PSMA 1007, differing from some prior Ga-PSMA studies.
Implications:
- F-PSMA 1007 PET/CT may offer enhanced preoperative insights into the etiology and histopathological characteristics of suspected RCC.
- This imaging modality could improve tumor staging accuracy for renal masses.
- F-PSMA 1007 PET/CT holds potential for theranostic applications in the management of renal cell carcinoma.
