Anterior diaphragmatic lymph nodes (ADLNs) are important in cancer staging. This study highlights the significance of laterally placed ADLNs, regardless of malignancy origin, for treatment planning.

Background:

  • Anterior diaphragmatic lymph nodes (ADLNs) are observed during CT scans for malignant disease.
  • While described, lymph nodes lateral to the cardiophrenic angles are less illustrated.
  • Understanding the entire ADLN group is crucial for accurate patient evaluation.

Purpose of the Study:

  • To review the anatomy of anterior diaphragmatic lymph nodes (ADLNs).
  • To emphasize the clinical importance of laterally placed ADLNs.
  • To assess the association of ADLN enlargement with various malignancies.

Main Methods:

  • Retrospective review of computed tomography (CT) scans in 125 patients with identified ADLNs.
  • Analysis of ADLN location, size changes on follow-up, and associated malignancies.
  • Comparison of lateral ADLN frequency with other ADLNs.

Main Results:

  • Lymphoma was the most common malignancy (41%) associated with enlarged ADLNs, followed by breast, colon, and lung cancer.
  • Laterally placed ADLNs were half as common as other ADLNs, with right-sided nodes being more frequent.
  • Changes in ADLN size were observed in 53 of 71 patients with follow-up CT scans.

Conclusions:

  • Enlarged ADLNs are associated with diverse malignancies, not exclusively one side.
  • Knowledge of the entire ADLN group, including lateral nodes, aids in radiotherapy planning and cancer staging.
  • ADLN evaluation is valuable for Hodgkin disease management and follow-up of other cancers.
