Background:

  • Essential tremor (ET) is a common movement disorder primarily affecting the upper limbs.
  • Orthostatic tremor (OT) is characterized by a high-frequency tremor that occurs specifically when standing.

Observation:

  • Three patients with ET presented with Heilman's orthostatic tremor features.
  • Electromyography revealed 6-8 Hz postural tremor in all limbs and synchronized 16 Hz discharges in legs upon standing.
  • Similar high-frequency rhythmic bursts were noted in upper limbs and spinal muscles in specific postures.

Findings:

  • Seven out of 12 patients with leg postural tremor showed altered muscle discharge frequency and synchronization when standing.
  • These patients reported mild unsteadiness and lower limb stiffness, without full OT syndrome.
  • Findings suggest a potential link between essential tremor and orthostatic tremor.

Implications:

  • Orthostatic tremor may be a variant or related entity within the spectrum of essential tremor.
  • A central derangement in motor activity organization, not solely related to standing, could be the cause.
  • Further research is needed to elucidate the relationship and underlying mechanisms of these tremors.
