Background:

  • Rheumatoid neutrophilic dermatitis (RND) is a rare extra-articular manifestation of rheumatoid arthritis (RA).
  • RND typically presents as symmetric skin lesions in patients with severe, long-standing RA.
  • It can occur in both seropositive and seronegative rheumatoid factor RA.

Observation:

  • A 65-year-old man with well-controlled RA developed RND.
  • The RND presented atypically as a unilateral manifestation on the right lower leg.
  • Skin biopsy revealed intense neutrophilic dermal infiltrate, microabscesses, and leukocytoclasis without vasculitis.

Findings:

  • The patient's skin lesions showed characteristic histopathological features of RND.
  • The unilateral presentation was unusual for this condition.
  • The patient experienced a positive response to intravenous methylprednisolone treatment.

Implications:

  • This case underscores the importance of considering RND in RA patients with unusual skin findings.
  • Atypical unilateral RND presentation can occur even in well-controlled RA.
  • Intravenous methylprednisolone may be an effective treatment for RND.
