Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

891
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.

Angle Closure Glaucoma: Treatment

1.0K
Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...

Glaucoma: Overview

1.1K
Glaucoma is an eye condition characterized by increased intraocular pressure that damages the retina and optic nerve, leading to irreversible blindness if left untreated. The human eye has various components, including the cornea, iris, pupil, lens, and optic nerve. Aqueous humor is secreted by the epithelium of the ciliary body in the posterior chamber and flows through the trabecular meshwork and canal of Schlemm, maintaining normal intraocular pressure. The trabecular meshwork and the canal...

Role of Matrix Metalloproteases in Degradation of ECM01:23

Role of Matrix Metalloproteases in Degradation of ECM

3.1K
Matrix metalloproteases (MMPs) are enzymes involved in the hydrolysis of proteins and glycoproteins of the extracellular matrix. MMPs are essential for the migration and proliferation of cells through the dense matrix network, throughout embryonic development, and throughout morphogenesis. The first MMP activity discovered was a collagenase in a tadpole's tail undergoing metamorphosis. The active collagen deposition and modifications lead to the morphogenesis of tadpoles into the adult...

Summary

Prostaglandin analogs increase matrix metalloproteinase (MMP) expression in the eye to lower intraocular pressure (IOP). A sustained-release implant may enhance this effect for longer glaucoma treatment.

Area of Science:

  • Ocular biology and pharmacology
  • Glaucoma research
  • Extracellular matrix remodeling

Background:

  • Matrix metalloproteinases (MMPs) degrade extracellular matrix (ECM), influencing tissue remodeling in health and disease.
  • Trabecular meshwork (TM) MMP activity is crucial for regulating intraocular pressure (IOP) via the conventional outflow pathway.
  • Imbalances in MMPs and tissue inhibitors of metalloproteinase (TIMPs) are implicated in glaucoma-related elevated IOP.

Purpose of the Study:

  • To investigate the mechanism by which prostaglandin analogs (PGAs) lower IOP.
  • To explore the role of MMPs in PGA-mediated ocular outflow enhancement.
  • To hypothesize the extended duration of action for a sustained-release PGA implant based on MMP upregulation.

Main Methods:

  • Review of in vivo and in vitro studies using animal models and ocular tissue cultures.
  • Analysis of PGA effects on MMP expression in the TM and ciliary body.
  • Hypothetical model for sustained-release bimatoprost implant's mechanism of action.

Main Results:

  • PGAs reduce IOP by increasing MMP expression in ocular tissues, promoting remodeling that enhances conventional and unconventional outflow.
  • PGA-induced MMP changes are dependent on drug identity and concentration.
  • Sustained-release bimatoprost is hypothesized to achieve higher tissue concentrations, leading to greater MMP upregulation and prolonged IOP reduction.

Conclusions:

  • PGA-induced MMP upregulation is a key mechanism for lowering IOP and enhancing ocular outflow.
  • Sustained-release formulations of PGAs may offer extended IOP control through enhanced and prolonged MMP-mediated tissue remodeling.
  • Further research into sustained-release PGA implants could provide a novel long-term glaucoma treatment strategy.
