Second-generation cryoballoon-based pulmonary vein isolation: Lessons from a five-year follow-up

Background:

• Second-generation cryoballoon (CB2) pulmonary vein isolation (PVI) shows promise for atrial fibrillation (AF) treatment.
• Long-term clinical outcome data for CB2-based PVI is limited.
• This study evaluates five-year outcomes of CB2-PVI in paroxysmal AF (PAF) and persistent AF (PersAF) patients.

Purpose of the Study:

• To assess the five-year clinical outcomes of CB2-based PVI.
• To compare outcomes between PAF and PersAF patients.
• To evaluate the impact of bonus freeze protocols on long-term success.

Main Methods:

• 139 patients (105 PAF, 34 PersAF) underwent index CB2-PVI with a 240-second freeze-cycle duration.
• A bonus freeze-cycle was applied in 71 patients; it was omitted in 68 patients.
• Clinical outcomes were assessed at a median follow-up of 60 months.

Main Results:

• Single-procedure success rate at five years was 54.4% (62% for PAF, 46.9% for PersAF).
• Overall success rate increased to 66.2% after repeat radiofrequency ablation procedures (69.2% for PAF, 65.6% for PersAF).
• No significant difference in outcomes was observed between bonus-freeze and no-bonus-freeze protocols (53.5% vs 57.4%).
