Summary

Geriatric education programs improve emergency physicians' knowledge and attitudes toward older adults. While improving recognition of geriatric syndromes, the program did not significantly alter medical decision-making in this population.

Background:

  • Emergency physicians (EPs) perceive care for older adults as complex due to insufficient geriatric skills.
  • A need exists to enhance geriatric competencies among EPs to improve care for elderly patients.

Purpose of the Study:

  • To evaluate the impact of a geriatric education program on EPs' knowledge, attitudes, and clinical practice concerning older adults.
  • To assess the effectiveness of case-based, interactive geriatric teaching for emergency medicine.

Main Methods:

  • A mixed-methods study involving 21 EPs from two Dutch hospitals.
  • Pre-post knowledge and attitude testing, retrospective chart review of 100 patients aged 70+, and qualitative interviews with 6 EPs.

Main Results:

  • The program significantly enhanced EPs' geriatric knowledge and their ability to recognize frailty and geriatric syndromes.
  • A significant improvement was observed in EPs' attention to the social history and circumstances of older patients (P=0.04).
  • No significant effect was found on medical decision-making; however, EPs valued the interactive, case-based teaching format.

Conclusions:

  • Geriatric emergency teaching benefits EPs, improving their knowledge and attentiveness to geriatric-specific factors.
  • Further research and program refinement are necessary to demonstrate sustained clinical practice improvements and positive patient outcomes.
