Background:

  • The role of implantable cardioverter-defibrillators (ICDs) in patients with sustained VT/VF during acute myocarditis is debated.

Purpose of the Study:

  • To compare the risk of major arrhythmic ventricular events (MAEs) in patients receiving ICDs for VT/VF during acute myocarditis versus those with VT/VF due to myocarditis sequelae.
  • To evaluate the long-term outcomes of ICD implantation in these distinct patient cohorts.

Main Methods:

  • Retrospective observational study conducted across 15 French university hospitals from 2007 to 2017.
  • Inclusion criteria: patients implanted with an ICD following sustained VT/VF during acute myocarditis or on myocarditis sequelae.

  • MAEs occurred in 39% of the acute myocarditis group and 60% of the myocarditis sequelae group.
  • One-year MAE rates were 19% (acute) and 43% (sequelae); three-year rates were 45% (acute) and 64% (sequelae).
  • Patients with VT/VF during acute myocarditis demonstrated a significantly high risk of recurrence.

Conclusions:

  • The risk of MAE recurrence remains substantial even after the resolution of acute myocarditis.
  • Sustained ventricular arrhythmias during the acute phase of myocarditis predict a very high risk of future VT/VF events.
  • These findings highlight the importance of careful monitoring and management strategies for patients with myocarditis-related ventricular arrhythmias.
