Summary

Recombinant human insulin-like growth factor-I (rhIGF-I) may improve bone density in individuals with anorexia nervosa (AN). rhIGF-I, combined with hormone therapy, significantly increased bone mineral density at the hip and spine with minimal side effects.

Background:

  • Growth hormone (GH) and insulin-like growth factor-I (IGF-I) are crucial for bone accrual, especially during puberty.
  • Undernutrition, as seen in anorexia nervosa (AN), leads to low IGF-I levels, impaired bone mineral density (BMD), and reduced bone strength.
  • Patients with AN often exhibit GH resistance, characterized by low IGF-I despite high GH levels.

Purpose of the Study:

  • To investigate the efficacy of recombinant human IGF-I (rhIGF-I) in improving bone health in adults with AN.
  • To evaluate the combined effects of rhIGF-I and hormone therapy (estrogen-progesterone) on bone density and turnover markers.

Main Methods:

  • A 9-month randomized controlled trial involving adults with AN.
  • Participants were assigned to four groups: rhIGF-I alone, estrogen-progesterone (EP) alone, rhIGF-I with EP, or neither.
  • Bone formation and resorption markers, BMD at the spine and hip, and side effects were monitored.

Main Results:

  • rhIGF-I administration increased bone formation markers.
  • EP administration decreased bone resorption markers.
  • The combination of rhIGF-I and EP resulted in significant increases in bone density at the spine and hip compared to the control group.
  • Side effects were minimal, with no significant hypoglycemia observed.

Conclusions:

  • rhIGF-I shows potential as a therapeutic agent for improving bone accrual in undernourished individuals with low IGF-I levels.
  • Combined therapy with rhIGF-I and hormone replacement may be an effective strategy for enhancing bone density in AN.
  • rhIGF-I appears to be safe for short-term use in this population.
