    Background:

    • A patient presented with retropharyngeal carcinoma requiring diagnostic and therapeutic airway interventions.
    • Pre-procedure evaluations, including barium swallow, did not reveal any signs of a tracheo-oesophageal fistula.

    Observation:

    • During laryngoscopy and oesophagoscopy under general anesthesia, the patient was intubated.
    • Positive airway pressure was applied during mechanical ventilation.

    Findings:

    • An abnormal communication, indicative of a tracheo-oesophageal fistula, became apparent only after intubation and positive pressure ventilation.
    • The fistula was not evident prior to the application of positive airway pressure.

    Implications:

    • This case highlights a rare complication of airway management in patients with retropharyngeal malignancy.
    • It underscores the importance of vigilance for developing tracheo-oesophageal fistulas during positive pressure ventilation.
    • Management strategies for such intraoperative complications should be considered.
