A rare case of acute aortic syndrome presented with multiple aortic pathologies including dissection, intramural hematoma, and penetrating ulcer. Surgical intervention led to a successful patient outcome.
Area of Science:
- Cardiovascular Surgery
- Thoracic Aortic Disease
- Diagnostic Imaging in Cardiology
Background:
- Acute aortic syndrome encompasses a spectrum of emergent aortic pathologies.
- Aortic dissection, intramural hematoma, and penetrating aortic ulcers are distinct but related conditions.
- Coexistence of multiple aortic entities is uncommon but clinically significant.
Observation:
- A patient presented with acute aortic syndrome exhibiting aortic dissection with intimal flap and true/false lumens.
- The patient also presented with concurrent intramural hematoma and a penetrating aortic ulcer.
- This constellation of findings is unusual, as typically only one entity is observed per patient.
Findings:
- The diagnostic imaging and pathological findings revealed a complex presentation of acute aortic syndrome.
- The patient's condition involved multiple, coexisting aortic pathologies.
- Surgical intervention involved an extended hemiarch replacement.
Implications:
- This case highlights the possibility of multiple acute aortic pathologies coexisting or evolving within a single patient.
- Accurate diagnosis and timely surgical management are crucial for favorable outcomes in complex aortic syndromes.
- Understanding the interplay between different aortic entities can improve diagnostic and therapeutic strategies.
