Background:

  • Complete transposition of the great arteries (TGA) is a critical congenital heart defect.
  • Associated cardiac anomalies, such as ventricular septal defects (VSDs), frequently complicate TGA management.
  • Posterior aorta is a rare anatomical variation in TGA.

Observation:

  • A case of TGA with a posterior aorta and multiple VSDs was encountered.
  • One perimembranous VSD was identified preoperatively and surgically closed during a Mustard operation.
  • Three additional muscular VSDs were not diagnosed preoperatively and presented surgical challenges.

Findings:

  • Multiple muscular VSDs can be challenging to detect preoperatively in complex TGA.
  • Missed VSDs can lead to significant intraoperative complications.
