Background:

  • Computerized adaptive testing (CAT) offers a potential solution to reduce this burden.

Purpose of the Study:

  • To evaluate the efficiency of CAT in improving the Veterans RAND 12 Item Health Survey (VR-12).
  • To assess CAT's ability to decrease patient question burden while maintaining outcome score accuracy.

Main Methods:

  • A previously developed CAT model was applied to 19,523 completed VR-12 surveys from orthopaedic clinics.
  • CAT and full survey scores for physical component summary (PCS) and mental component summary (MCS) were calculated.
  • Accuracy was compared using statistical analyses including correlation coefficients and Bland-Altman assessment.

Main Results:

  • CAT reduced the number of questions by 33% (4 fewer questions per subject).
  • CAT and full VR-12 scores showed high correlation (Pearson's r = 0.97 for PCS, 0.98 for MCS) and agreement (ICC = 0.96 for PCS, 0.97 for MCS).
  • Score differences were within the minimal clinically important difference (MCID) for over 95% of cases.

Conclusions:

  • CAT application to the VR-12 survey effectively reduces patient response burden.
  • The CAT method maintains the integrity and accuracy of the outcome scores.
  • CAT presents a viable solution for efficient patient-reported outcome measurement in orthopaedics.
