Summary

Hypertensive choroidopathy secondary to hypertensive disorders of pregnancy (HDP) can improve with treatment. Spectral-domain optical coherence tomography (SD-OCT) revealed increased choroidal thickness resolving before serous retinal detachment.

Background:

  • Hypertensive disorders of pregnancy (HDP) can lead to serious maternal and fetal complications.
  • Hypertensive choroidopathy is a rare but significant ocular manifestation of HDP.

Observation:

  • A 27-year-old woman with HDP and disseminated intravascular coagulation (DIC) developed visual disturbances post-cesarean delivery.
  • Ophthalmic imaging revealed serous retinal detachment (SRD), granular leakage on fluorescein angiography (FA), choroidal hypoperfusion/hyperpermeability on indocyanine green angiography (IA), and increased choroidal thickness on SD-OCT/EDI-OCT.

Findings:

  • Treatment of hypertension and DIC led to gradual resolution of SRD and improvement in visual symptoms.
  • SD-OCT demonstrated that increased choroidal thickness in affected areas improved earlier than complete resolution of SRD.

Implications:

  • This case highlights the potential for visual recovery in hypertensive choroidopathy associated with HDP with appropriate systemic management.
  • SD-OCT offers a valuable tool for monitoring choroidal structural changes and aiding diagnosis and follow-up in hypertensive choroidopathy.
