Summary

A modified posterior vertebral column resection (PVCR) technique may reduce neurological complications in severe spinal deformities. This modified PVCR approach appears safer than the conventional method, though further research is needed.

Background:

  • Posterior vertebral column resection (PVCR) is advantageous for severe, rigid spinal deformities but has a high complication rate.

Purpose of the Study:

  • To evaluate the outcomes of PVCR in severe, rigid spinal deformities.
  • To present a modified PVCR technique and compare its efficacy and safety against the conventional method.

  • A retrospective cohort study of 38 patients with severe, rigid spinal deformities (deformity >90°, flexibility index <20%).
  • Patients were divided into two groups: conventional PVCR (n=21) and modified PVCR (n=17).
  • The modified technique preserves posterior elements until deformity correction to prevent ventral settling and reduce spinal cord handling.

Main Results:

  • Mean coronal correction was 50% and sagittal correction was 52.4%.
  • No significant differences in operating time or blood loss were observed between techniques.
