Introduction
Exercise stress testing, commonly known as a treadmill test, is a noninvasive procedure used to evaluate cardiovascular function and diagnose heart conditions.
Definition
An exercise stress test measures the heart's response to exertion using a treadmill or stationary bicycle. Chest electrodes record the heart's electrical activity through an ECG, and blood pressure is monitored regularly.

Purpose of the Study:

  • To compare peak exercise responses between individualized ramp and modified Bruce protocols in elderly patients.
  • To evaluate differences in workload, duration, and perceived exertion during treadmill testing.

Main Methods:

  • 40 elderly patients (≥70 years) without prior heart disease underwent two consecutive days of treadmill testing.
  • Tests included modified Bruce protocol and individualized ramp protocol.
  • Peak exercise parameters (heart rate, blood pressure, duration, metabolic equivalents) and perceived exertion (Borg scale) were recorded.

Main Results:

  • The ramp protocol resulted in longer exercise duration and higher achieved workload compared to the modified Bruce protocol.
  • Patients reported significantly lower perceived exertion (Borg scale 5.6±1.4) with the ramp protocol versus the modified Bruce protocol (8.7±1.4).

Conclusions:

  • The individualized ramp protocol is superior for optimizing test duration, workload, and patient comfort in elderly individuals undergoing treadmill testing.
  • This suggests the ramp protocol may provide a more accurate and comfortable assessment of exercise capacity in this population.
