    Background:

    • Gram-negative bacillary pneumonia is a serious complication in cancer patients, particularly those with acute leukemia.
    • Clinical and radiological features can be subtle, with fever being the most common sign.

    Purpose of the Study:

    • To analyze the clinical and radiological characteristics of gram-negative bacillary pneumonia in adult cancer patients.
    • To identify common pathogens, treatment outcomes, and factors influencing cure rates.

    Main Methods:

    • Retrospective analysis of 217 pneumonia episodes in 189 adult cancer patients (Nov 1968-Dec 1974).
    • Evaluation of clinical symptoms, radiological findings, causative organisms, and treatment responses.

    • Fever (>=101°F) was the most frequent symptom (90%). Radiographic evidence was present in 83%, often showing alveolar infiltrates. Klebsiella and Pseudomonas were common pathogens.
    • Overall cure rate was 61%, higher for Klebsiella (70%) and Pseudomonas (64%) infections. Pulmonary abscesses occurred in 14%.
    • Cure rates correlated with antibiotic sensitivity and neutrophil count. Gentamicin, sisomicin, tobramycin, amikacin, or combinations showed best results.

    Conclusions:

    • Early and aggressive antibiotic therapy, guided by sensitivity testing and patient's neutrophil count, significantly improves outcomes for gram-negative bacillary pneumonia in cancer patients.
    • Appropriate antibiotic selection, including newer aminoglycosides and combinations, is crucial for managing this severe infection.
