  • Familial hypercholesterolaemia (FH) is an underdiagnosed genetic condition requiring early intervention.

Purpose of the Study:

  • To evaluate the impact of reflex automated laboratory comments on primary care lipid results.
  • To assess the effectiveness of these comments in increasing referrals to a specialized Familial Hypercholesterolaemia service.

Main Methods:

  • Reflex automated comments, including direct referral recommendations, were provided on primary care lipid results in one Clinical Commissioning Group (CCG).
  • GP referrals to the West Midlands Familial Hypercholesterolaemia service (WMFHS) from the intervention CCG were compared over two years with four similar comparator CCGs without the comments.

Main Results:

  • The intervention CCG (Wolverhampton) had significantly more referrals to WMFHS (241) compared to any single comparator CCG (range 8-65).
  • Referrals from Wolverhampton (241) exceeded the combined referrals (172) from all four comparator CCGs.

Conclusions:

  • Targeted reflex laboratory comments can influence clinical management and patient identification.
  • Automated comments on lipid results show potential for improving the diagnosis of familial hypercholesterolaemia.
