Background:

  • Synchronous diagnosis of acute myeloid leukemia (AML) and symptomatic multiple myeloma (MM) presents significant therapeutic challenges due to the rarity and complexity of managing two distinct hematologic malignancies concurrently.
  • Effective treatment strategies for patients with both AML and MM are not well-established, necessitating case reports to guide clinical practice.

Observation:

  • A 68-year-old male presented with simultaneous symptomatic multiple myeloma and acute myeloid leukemia.
  • Initial treatment with 7+3 induction followed by azacitidine plus lenalidomide maintenance resulted in a 40-month response for both malignancies.
  • Subsequent relapse of multiple myeloma was treated with azacitidine plus daratumumab, achieving an additional 15 months of progression-free survival.

Findings:

  • The combination of azacitidine and lenalidomide, followed by azacitidine and daratumumab, demonstrated efficacy in managing synchronous AML and symptomatic MM.
  • Despite treatment, a minimal reduction in the myeloid clone size was observed over time, suggesting potential for residual disease.
  • This case highlights the feasibility of managing concurrent AML and symptomatic MM with appropriate therapeutic combinations.

Implications:

  • Simultaneous treatment of AML and symptomatic MM is achievable with carefully selected combination regimens.
  • Further research into optimal sequencing and combination therapies for synchronous hematologic malignancies is warranted.
  • Case studies like this are crucial for developing evidence-based guidelines for rare and complex hematologic conditions.
