Background:

  • Hypertension affects over a billion people globally, disproportionately impacting low- and middle-income countries (LMICs).
  • Risk factors include population growth, aging, genetics, and lifestyle choices like poor diet, inactivity, obesity, and stress.
  • Untreated hypertension leads to severe cardiovascular events, including heart disease, stroke, kidney failure, and premature death.

Purpose of the Study:

  • To evaluate the effectiveness of various hypertension screening strategies (mass, targeted, opportunistic).
  • To determine if these screening methods reduce hypertension-related morbidity and mortality.

Main Methods:

  • A comprehensive literature search was conducted across multiple databases (Cochrane, MEDLINE, Embase, LILACS, ClinicalTrials.gov, WHO ICTRP) up to April 2020.
  • Included studies were randomized controlled trials (RCTs) and non-RCTs (NRCTs) such as controlled before-and-after, interrupted time series, and cohort studies.
  • Data extraction, risk of bias assessment, and synthesis (meta-analysis or narrative) were planned for eligible studies.

Main Results:

  • A total of 9,335 titles and abstracts were screened.
  • 54 studies were identified for full-text review.
  • Critically, no studies met the eligibility criteria for inclusion in the analysis.

Conclusions:

  • The assumption that early hypertension detection through screening reduces morbidity and mortality lacks testing in rigorous research.
  • High-quality evidence from RCTs or NRCTs on the effectiveness, costs, or harms of different hypertension screening strategies is currently unavailable.
