  • Symptoms began three days after completing a half-marathon.

  • A presumptive diagnosis of AIP was established based on clinical presentation and radiological findings.
  • The patient received high-dose corticosteroids and mycophenolate mofetil.
  • Successful treatment led to recovery from AIP.

  • Early diagnosis and combined immunosuppressive therapy may improve outcomes in AIP.
  • Further research into exercise-induced AIP and optimal treatment strategies is warranted.
