Background:

  • Penile melanomas are extremely rare, comprising less than 1% of all melanoma cases.
  • The rarity of penile melanoma contributes to a lack of standardized treatment protocols.

Observation:

  • A 59-year-old male presented with tumorous lesions on the prepuce and a subcutaneous lesion on the corpus cavernosum.
  • Initial differential diagnoses included Merkel carcinoma, dermatofibrosarcoma protuberans, lymphoma, and melanoma.
  • Histopathology and immunohistochemistry confirmed achromatic malignant melanoma of the prepuce with in-transit metastasis.

Findings:

  • The patient was diagnosed with achromatic preputial malignant melanoma and an in-transit metastasis.
  • A planned penectomy was recommended due to the advanced stage of the disease.

Implications:

  • Delayed diagnosis significantly worsens the prognosis for penile melanoma patients.
  • Standardizing histopathologic evaluation post-excision is essential to prevent advanced disease and radical surgical interventions like penectomy.
