Background:

  • The Prostate Cancer gene 3 (PCA3) urine test is increasingly used for prostate cancer (PC) diagnosis.
  • Previous research suggested PCA3 is unaffected by inflammation, but evidence was limited.

Purpose of the Study:

  • To investigate the effect of histological inflammation on PCA3 scores.
  • To determine if inflammation influences the accuracy of PCA3 testing for prostate cancer.

Main Methods:

  • PCA3 scores were measured in patients before prostate biopsy and radical prostatectomy.
  • Inflammation was assessed and quantified in patients without PC.
  • Univariate and multivariate analyses examined the relationship between inflammation and PCA3 scores.

Main Results:

  • PCA3 scores were lower before prostatectomy compared to before positive biopsy.
  • A significant proportion of negative biopsies (78%) showed inflammation.
  • PCA3 scores decreased progressively with increasing grade and extent of inflammation (p=0.016).

Conclusions:

  • PCA3 scores are reduced in the presence of inflammation.
  • Inflammation is a significant factor influencing PCA3 test results, particularly after recent biopsies.
  • Inflammation should be considered when interpreting PCA3 and other prostate cancer diagnostic tests.
