Summary

Patient-specific endovascular simulation did not lower complication rates. However, a structured simulation curriculum for novice angiographers significantly reduced radiation exposure and fluoroscopy time during diagnostic neuroangiography.

Area of Science:

  • Medical Education
  • Interventional Radiology
  • Neurosurgery

Background:

  • Endovascular simulation is a validated training tool.
  • Evidence for direct patient benefit, such as reduced complications, remains limited.
  • The impact of patient-specific rehearsal and structured curricula on outcomes is under investigation.

Purpose of the Study:

  • To evaluate the impact of patient-specific endovascular simulation rehearsal on complication rates.
  • To assess the effectiveness of a structured simulation curriculum for novice angiographers.
  • To determine if simulation training improves patient outcomes in diagnostic neuroangiography.

Main Methods:

  • A randomized trial involving 40 patients undergoing diagnostic neuroangiography.
  • Training group: patient-anatomy-specific rehearsal on a high-fidelity simulator prior to the procedure.
  • Comparison of novice angiographers: structured simulation curriculum vs. traditional skill acquisition.
  • Outcome measures included radiation exposure, fluoroscopy time, contrast agent volume, and silent cerebral ischemia (DWI).

Main Results:

  • Patient-specific rehearsal showed no significant difference in DWI lesions or other quality measures.
  • A structured simulation curriculum significantly reduced fluoroscopy time and radiation exposure for novice angiographers.
  • The simulation curriculum did not impact total procedure duration, contrast volume, or catheter usage.

Conclusions:

  • Patient-specific endovascular simulation rehearsal did not demonstrate measurable benefits for unselected patients.
  • Structured simulation-based training effectively reduces radiation dose and fluoroscopy time in novice angiographers' initial procedures.
  • Simulation training holds promise for improving efficiency and safety in endovascular procedures.
