Area of Science:

  • Virology
  • Molecular Diagnostics
  • Infectious Diseases

Background:

  • Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) has caused a global pandemic with significant mortality.
  • The virus exhibits broad tropism, with detection reported in diverse biological specimens.

Purpose of the Study:

  • To assess the accuracy and sensitivity of a modified CDC-based SARS-CoV-2 assay (N1 and N2 targets).
  • To evaluate the impact of different sample matrices on the assay's analytical sensitivity.

Main Methods:

  • Quantitative reverse transcription-polymerase chain reaction (qRT-PCR) was employed.
  • The study compared SARS-CoV-2 detection in nasal swabs (VTM), bronchoalveolar lavage (BAL), sputum, plasma, cerebrospinal fluid (CSF), stool, phosphate-buffered saline (PBS), and Hanks' Balanced Salt Solution (HBSS).
  • Limits of detection (LoD) were determined for each specimen type and target gene.

Main Results:

  • Established LoDs ranged from 10-20 copies/reaction for N1 and 1-10 copies/reaction for N2.
  • Alternative transport media like PBS and HBSS showed comparable LoDs to viral transport medium (VTM).
  • The N2 gene target exhibited the highest sensitivity in CSF samples.

Conclusions:

  • A modified CDC-based assay provides accurate SARS-CoV-2 detection.
  • The test demonstrates consistent sensitivity across a wide array of tested sample types, supporting its broad clinical utility.
