Summary

Radiation therapy after prostatectomy modestly worsens urinary quality of life, particularly incontinence. Delaying radiation beyond one year post-surgery may lead to greater urinary function decline in prostate cancer patients.

Purpose of the Study:

  • To evaluate the impact of radiation therapy on urinary quality of life in prostate cancer patients following prostatectomy.
  • To assess how the timing of radiation therapy relative to prostatectomy affects urinary outcomes.

Main Methods:

  • 106 patients receiving post-prostatectomy radiation therapy were assessed.
  • The Expanded Prostate Cancer Index Composite (EPIC) questionnaire was used to measure urinary quality of life.
  • Data were collected before radiation and at a 2-year follow-up, with analysis stratified by time from prostatectomy to radiation.

Main Results:

  • A statistically significant worsening of the urinary domain summary score was observed 2 years after radiation therapy (p=0.0085).
  • Urinary function, bother, and incontinence subscales also showed significant declines, with incontinence exhibiting the most substantial change.
  • Patients receiving radiation therapy more than 1 year after prostatectomy experienced a significant worsening of the urinary summary score, unlike those treated within 1 year.

Conclusions:

  • Post-prostatectomy radiation therapy is linked to modest decreases in urinary quality of life.
  • Greater urinary quality of life deterioration was noted in patients treated more than 1 year after prostatectomy.
  • These findings suggest no benefit to delaying radiation therapy beyond the immediate postoperative period.
