Background:

  • Adenoidectomy is a common procedure for pediatric sleep-disordered breathing.
  • Both hot (suction diathermy) and cold (curettes) methods are used, with similar short-term outcomes.
  • Long-term efficacy of each method on sleep symptoms remains unclear.

Purpose of the Study:

  • To compare the one-year outcomes of hot versus cold adenoidectomy.
  • To evaluate the impact of each technique on pediatric sleep disorder symptoms.

Main Methods:

  • A prospective, randomized, single-blinded study.
  • 58 children under 16 underwent either hot or cold adenoidectomy.
  • Pediatric Sleep Questionnaire (PSQ) scores were assessed at one month and one year post-surgery.

Main Results:

  • No significant differences in PSQ scores were observed at one month.
  • At one year, hot adenoidectomy showed a significantly greater improvement in PSQ scores (+0.31) compared to cold adenoidectomy (+0.22; p=0.009).
  • Complication rates and adenoid size were similar between groups.

  • Hot adenoidectomy is associated with superior long-term improvement in sleep disorder symptoms compared to the cold method.
  • The findings suggest a potential benefit of the hot technique for sustained symptom relief.
