Summary

Surgical resection of primary pulmonary sarcoma (PPS) in selected patients, alongside multimodality therapy, can lead to good long-term outcomes. Tumor size and type of lung resection are key prognostic factors for survival.

Background:

  • Primary pulmonary sarcoma (PPS) is a rare malignancy, representing less than 1.1% of all lung tumors.
  • Limited outcome data exist for PPS, necessitating further investigation into prognostic factors.

Purpose of the Study:

  • To evaluate the outcomes of patients who underwent surgical resection for PPS.
  • To identify prognostic factors influencing survival and recurrence in primary pulmonary sarcoma.

Main Methods:

  • Retrospective review of patients undergoing resection for PPS between 2002 and 2018.
  • Analysis of survival, disease-free survival, and recurrence based on patient demographics, tumor characteristics (size, grade, stage), and surgical factors (resection type, completeness of resection).

Main Results:

  • Thirteen patients were included, with a median tumor size of 11.5 cm.
  • Overall 5-year survival was 60%, with a median disease-free survival of 17 months.
  • Tumor size, gender, and type of lung resection were significant predictors of survival. STS T stage and lung cancer TNM stage predicted recurrence.

Conclusions:

  • Surgical resection, as part of a multimodality treatment strategy in carefully selected patients, can achieve favorable long-term outcomes for primary pulmonary sarcoma.
  • Prognostic factors such as tumor size and surgical approach are critical for predicting patient outcomes.
