Background:
The appearance of diffuse lentigines in children is rare and a genetic syndrome should initially be envisaged. Another little-known cause of photo-distributed lentigines is use of voriconazole. We present a new case in which the original feature is the very short time of onset.
Patients And Methods:
A 9-year-old immunosuppressed girl treated with voriconazole for 3 months presented lentigines in exposed areas after only 4 weeks of exposure.
Discussion:
The literature contains only around ten cases of photo-distributed lentigines under voriconazole in children. The condition can appear very early, as in our case. Voriconazole also appears to induce cutaneous squamous cell carcinomas and even melanoma. The benefit-risk ratio of prescribing this drug must therefore be carefully evaluated, and close clinical monitoring and photoprotection must be instituted.
Insights
Voriconazole can cause sun-exposed skin spots (lentigines) in children, appearing rapidly. This rare side effect necessitates careful drug risk assessment and sun protection for pediatric patients.
Area of Science:
- Pediatric Dermatology
- Pharmacovigilance
- Mycology
Background:
- Diffuse lentigines in children are rare, often suggesting genetic syndromes.
- Voriconazole, an antifungal, is a less common cause of photo-distributed lentigines.
- This report highlights a unique case with an exceptionally short onset of symptoms.
Observation:
- A 9-year-old immunosuppressed female developed lentigines on sun-exposed skin.
- The patient was undergoing treatment with voriconazole for 3 months.
- Lentigines appeared after only 4 weeks of voriconazole exposure.
Findings:
- The case adds to a small number of reported pediatric instances of voriconazole-induced photo-distributed lentigines.
- The onset of this side effect can be very rapid.
- Voriconazole is also linked to increased risks of cutaneous squamous cell carcinomas and melanoma.
Implications:
- The benefit-risk ratio of prescribing voriconazole in children requires careful evaluation.
- Close clinical monitoring for skin changes is crucial in pediatric patients on voriconazole.
- Strict photoprotection measures are essential for children treated with voriconazole.
