Non-invasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) is a newly classified thyroid tumor. This classification aims to prevent overtreatment of indolent tumors, improving patient management.
Area of Science:
- Endocrinology
- Surgical Pathology
- Oncology
Background:
- Papillary thyroid carcinoma (PTC) incidence rose, but mortality remained static.
- Indolent PTC subtypes, often encapsulated, were overtreated with total thyroidectomy and radioactive iodine (RAI).
- A reclassification was needed to distinguish these indolent tumors from aggressive PTC.
Purpose of the Study:
- To review the emergence of the term Non-invasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP).
- To discuss the histological and molecular background of NIFTP.
- To highlight the rationale for conservative management of NIFTP.
Main Methods:
- Review of literature on thyroid tumor classification and diagnosis.
- Analysis of histological criteria for NIFTP diagnosis.
- Examination of the genetic basis and molecular markers relevant to NIFTP.
Main Results:
- NIFTP reclassification addresses indolent tumors previously diagnosed as PTC.
- Strict histological criteria are now essential for NIFTP diagnosis.
- Ongoing research focuses on molecular markers to support NIFTP diagnosis.
Conclusions:
- NIFTP represents a distinct entity allowing for less aggressive treatment strategies.
- Accurate histological diagnosis is crucial to avoid overtreatment.
- Further research into molecular markers may refine NIFTP diagnosis and management.
