Background:

  • Common Variable Immunodeficiency (CVID) impairs antibody production, increasing susceptibility to bacterial infections.
  • Lung disease is a primary cause of morbidity and mortality in CVID patients.

Purpose of the Study:

  • To analyze clinical, radiological, and functional characteristics of lung disease in CVID patients.
  • To determine the prevalence and patterns of lung involvement in CVID.

Main Methods:

  • Retrospective analysis of 80 CVID patients from 1982-2018.
  • Evaluation of clinical data, lung CT scans, and pulmonary function tests.
  • Histopathological analysis for select cases.

Main Results:

  • 55% of patients had abnormal lung CT scans.
  • Bronchiectasis (36.4%) and GLILD (47.3%) were the most common findings.
  • GLILD patients showed normal or restrictive lung function patterns; biopsy confirmed LIP in 19%.

Conclusions:

  • Granulomatous-Lymphocytic Interstitial Lung Disease (GLILD) is the predominant lung disease in CVID.
  • CT scans are useful for diagnosis, but functional tests are better for monitoring CVID lung disease.
  • Biopsy and treatment are indicated for unclear diagnoses or disease progression.
