Background:

  • Hemispatial neglect, both clinical and subclinical, is linked to functional disability after stroke.
  • Current assessment methods may not adequately identify subtle, or subclinical, forms of neglect.

Purpose of the Study:

  • To investigate if increasing task complexity enhances the sensitivity of neglect assessment.
  • To determine if complex dual-tasks can identify subclinical hemispatial neglect.

Main Methods:

  • Developed and compared simple and complex computerized dual-tasks on a large screen.
  • Assessed 40 stroke patients (20 left hemisphere, 20 right hemisphere) and 20 healthy controls.
  • Utilized the Bells Test for traditional neglect assessment.

Main Results:

  • Right hemisphere stroke patients showed more left hemifield errors on dual-tasks; left hemisphere patients did not differ from controls.
  • No significant group differences were found using the Bells Test.
  • Complex dual-tasks increased reaction times and revealed slower responses to left-sided stimuli across all groups.

Conclusions:

  • Large-screen dual-tasks are sensitive in detecting subclinical neglect in stroke survivors.
  • There is a need for novel, sensitive, and ecologically valid methods to assess subclinical neglect.
