Drug Dosing: Geriatric Patients01:15

Drug Dosing: Geriatric Patients

159
Elderly individuals encompass a diverse population with varying degrees of age-related physiological changes. Defining the elderly presents challenges, as the geriatric population is often arbitrarily categorized as individuals older than 65. However, many individuals in this group lead active and healthy lives, with an increasing number surpassing 85 years and falling into the older elderly category. Physiological changes associated with aging impact performance capacity and homeostatic...

Restorative Care01:19

Restorative Care

2.2K
Restorative care is provided once a patient has been discharged from a healthcare facility and requires additional services. The additional services include home care, rehabilitation programs, and extended care. Restorative care centers help the patient regain their previous level of functioning or acquire a new level of functioning due to the incapacitating effects of a disease or a disability. It aims to assist patients in enhancing their quality of life by encouraging independence,...

Prescription, Nonprescription and Orphan Drugs01:02

Prescription, Nonprescription and Orphan Drugs

1.1K
Prescription drugs require a prescription from a medical practitioner and can only be obtained from a pharmacy. They have many applications, including treating pain, anxiety, and hypertension.
The misuse and addiction to prescription drugs is a growing problem that can affect people of all age groups, specifically teenagers. This can happen when prescription medications are used in ways not intended by the prescriber, such as taking someone else's prescription or using medication for...

Effect of Hepatic Disease on Pharmacokinetics: Dose Adjustments Due to Hepatic Impairment

128
Hepatic impairment, characterized by decreased liver function, does not uniformly mandate adjustments in drug dosage. Whether dosage modifications are necessary depends on various factors related to the drug's metabolism and elimination pathways. If a drug is primarily excreted via the kidneys and bypasses significant hepatic processing, if it undergoes minimal metabolic transformation in the liver, or if it is volatile and primarily expelled through the lungs, dose adjustments may not be...

Specialized Care Centers and Settings-I01:30

Specialized Care Centers and Settings-I

1.3K
Specialized care settings or centers are situated in convenient locations within the community and offer care to a specific group or population. They consist of daycare facilities, mental health facilities, rural health facilities, educational institutions, industries, shelters for the homeless, and rehabilitation facilities.
Daycare centers
They provide several functions. Some facilities care for healthy newborns and children whose parents work, while others are medically focused and care for...

Summary

Potentially inappropriate medications (PIMs) in older adults decreased, but psychotropic drug use increased. Optimizing prescriptions for proton pump inhibitors (PPIs) and psychotropics is crucial for deprescribing polypharmacy in intermediate care.

Area of Science:

  • Geriatrics
  • Clinical Pharmacy
  • Health Services Research

Background:

  • Inappropriate prescribing of medications for older adults is a significant global healthcare challenge.
  • Potentially Inappropriate Medications (PIMs) contribute to adverse health outcomes in the elderly population.

Purpose of the Study:

  • To determine the prevalence of PIMs in older patients at admission and discharge.
  • To evaluate changes in medication prescribing patterns in an intermediate care facility.

Main Methods:

  • A consecutive sample of 100 patients aged 65 years and older was enrolled.
  • Potentially Inappropriate Medications (PIMs) were identified using the 2015 Beers criteria.
  • Medication data was collected at admission and discharge.

Main Results:

  • A significant reduction in the prescription of drugs to avoid and proton pump inhibitors (PPIs) was observed.
  • There was a significant increase in patients receiving at least one psychotropic drug to avoid or use with caution.
  • Inappropriate PPI prescription was primarily linked to concurrent heparin use.

Conclusions:

  • Deprescribing inappropriate polypharmacy in intermediate care facilities requires optimization of PPI and psychotropic drug prescriptions.
  • Further research into deprescribing strategies for PIMs in older adults is warranted.
