Near-fatal poisoning after ricin injection

Intentional ricin injection from castor beans caused near-fatal poisoning. Supportive care successfully treated severe systemic inflammatory response syndrome, multiple organ dysfunction, and necrotizing fasciitis in a young man.

- The patient received approximately 738 µg of ricin A-chain, resulting in severe systemic toxicity.
- Laboratory findings revealed hypoalbuminemia, proteinuria, thrombocytopenia, leukocytosis, elevated cytokines, troponin, and creatine kinase.
- Successful management involved aggressive fluid resuscitation, vasopressors, broad-spectrum antibiotics, surgical debridement, and skin grafting.
