State-Independent Microstructural White Matter Abnormalities in Major Depressive Disorder

Major depressive disorder (MDD) shows persistent brain abnormalities in the insula, regardless of symptom state. Other brain region impairments appear to be state-dependent in MDD patients.

- Diffusion tensor imaging (DTI) was used to assess whole-brain fractional anisotropy (FA) in 127 medication-free MDD patients and 118 healthy controls (HCs).
- 62 remitted MDD patients underwent a second DTI scan after 6 months of treatment.
- Independent t-tests, controlling for gender, age, and education, compared FA values between groups.
Main Results:
- MDD patients exhibited significant FA reductions in the left insula, left middle occipital gyrus, right thalamus, left pallidum, and left precuneus compared to HCs.
- Remitted MDD patients showed significant FA reductions specifically in the left insula compared to HCs.
- No significant FA differences were found between current MDD and remitted MDD patients.
Conclusions:
- Insula abnormalities in MDD appear to be state-independent, persisting even after remission.
- Abnormalities in the middle occipital gyrus, thalamus, pallidum, and precuneus seem to be state-dependent, potentially improving with treatment.
