Rocky Mountain Spotted Fever (RMSF) is a severe tick-borne illness caused by Rickettsia rickettsii, a Gram-negative, coccobacillary bacterium. This pathogen is an obligate intracellular parasite, requiring a host cell for replication. Transmission occurs through the bite of an infected tick. In the United States, the most important vectors are Dermacentor variabilis (American dog tick) and Dermacentor andersoni (Rocky Mountain wood tick), though other tick species may also serve as vectors.

Background:

  • While five cases of meningococcal disease with complement deficiency have been reported in Korea, C6 deficiency has not been previously documented.

Observation:

  • A 21-year-old police trainee experienced recurrent episodes of meningococcal meningitis.
  • The patient lived in communal quarters, increasing exposure risk.

Findings:

  • The patient was diagnosed with meningococcal meningitis attributed to C6 deficiency.
  • Treatment involved intravenous ceftriaxone and a post-discharge meningococcal conjugate vaccine booster.

Implications:

  • This case represents the first documented instance of C6 deficiency in Korea.
  • Routine screening for complement deficiencies is crucial for managing recurrent meningococcal infections and preventing future occurrences.
