Background:

  • Leiomyomas, commonly known as fibroids, are the most frequent benign tumors in the female reproductive system, typically originating in the uterus.
  • While uterine location is most common, leiomyomas can develop in other areas, including the ovaries, broad ligament, and exceptionally, the abdominal wall.

Observation:

  • A patient presented with abnormal uterine bleeding, right upper quadrant abdominal pain, and an ovarian mass, initially considered for laparoscopy.
  • Laparotomy was necessitated due to an abdominal wall mass obstructing veress needle insertion for laparoscopy.
  • Pathological examination confirmed the abdominal mass to be a leiomyoma.

Findings:

  • The case presents a rare instance of a leiomyoma occurring in the anterior abdominal wall.
  • The diagnosis of abdominal wall leiomyoma should be considered in the differential diagnosis of abdominal masses, even without a prior myomectomy history.

Implications:

  • This case underscores the potential for leiomyoma formation in extragenital sites containing smooth muscle, such as the anterior abdominal wall.
  • Recognition of this possibility is crucial for accurate diagnosis and appropriate management of abdominal masses.
