A military member experienced chronic peroneal tendon dislocation after a sports injury. Surgical repair led to pain resolution and full duty return within 4.5 months.

Background:

Chronic peroneal tendon dislocation is a rare condition often resulting from sports-related injuries.
Accurate diagnosis is crucial for effective management and patient recovery.

Observation:

A 47-year-old active duty military member presented with lateral ankle pain following a sports injury.
Magnetic resonance imaging (MRI) revealed superficial peroneal retinaculum (SPR) disruption, a flattened retrofibular groove, peroneus longus tendon dislocation, and peroneus brevis tendon tear.

Findings:

The patient underwent surgical intervention including fibular groove deepening, SPR reconstruction, peroneus brevis debridement, and peroneus longus tubulurization.
No intraoperative or perioperative complications were noted.
