The T allele of the IRAK1 gene (rs1059703) significantly increases rheumatoid arthritis (RA) risk and disease severity in Iranian patients. This genetic factor also influences the age of RA onset, highlighting its role in disease progression.

Background:
- Rheumatoid arthritis (RA) is a chronic autoimmune disease.
- Interleukin-1 receptor-associated kinase (IRAK1) and cluster of differentiation 40 (CD40) are implicated in RA pathogenesis.
- Previous studies on IRAK1 and CD40 gene polymorphisms in RA have yielded conflicting results.
Purpose of the Study:
- To investigate the association of specific IRAK1 and CD40 gene polymorphisms (rs1059703, rs4810485, rs1883832) with rheumatoid arthritis risk.
- To provide a more accurate estimation of the contribution of these polymorphisms to RA susceptibility.
- To explore the correlation between IRAK1 rs1059703 genotypes and clinical/laboratory features of RA patients.
Main Methods:
- A case-control study involving 200 RA patients and 200 healthy controls.
- Genomic DNA extraction followed by genotyping of three single nucleotide polymorphisms (SNPs) using PCR-RFLP.
- Statistical analysis using chi-square test to evaluate genotype and allele frequencies and their association with RA.
Main Results:
- The T allele and TT/CT genotypes of IRAK1 rs1059703 polymorphism were significantly associated with increased rheumatoid arthritis risk (OR=2.36, P=0.0001 for T allele).
- No significant association was found between CD40 gene polymorphisms (rs1883832, rs4810485) and RA susceptibility.
- IRAK1 rs1059703 genotypes showed significant differences in relation to medical and laboratory features of RA patients.
Conclusions:
- The rs1059703 T allele of the IRAK1 gene is a risk factor for rheumatoid arthritis in the Iranian population.
- This polymorphism is associated with increased RA risk, disease severity, and earlier onset age.
- CD40 gene polymorphisms studied do not appear to contribute significantly to RA risk in this cohort.
